Our Patient Intake Forms are a vital part of any treatment and we would encourage you to print out the relevant forms, complete them at home prior to seeing one of our Chiropractors, and bring them in with you for your consultation. We do have printed copies at reception should you forget but in the interest of saving time and ensuring we run to schedule we would really appreciate it if you could bring the completed forms in with you.

New Patient Intake Form

If you are a new patient to the practice and have never seen one of the Chiropractors before then please complete a New Patient Intake Form

Presenting Complaint

In order to get a better idea of your presenting complaint we make use of a questionnaire which includes a body chart and a few questions relating to your area of concern.

COVID-19 Consent to Treat

A COVID-19 informed consent form is required to be completed for each and every visit, regardless of how many times you have seen your Chiropractor or when your last consultation was. Please ensure you print this and complete it BEFORE your consultation. Please remember to bring the completed form in with you.

COVID-19

These are extraordinary times and with that comes the responsibility of all of us to do as much as we can to protect ourselves and to protect one another. As a practice we have guidelines in place including screening of our patients before consultations, spacing of appointments, rigid hygiene practices and general mindful practice in line with the recommendations by the Government, our governing body and our professional association. So what can you do to help? Get educated on what COVID-19 is, take the necessary precautions to limit the spread, and continue to wash your hands thoroughly and use your face mask at all times!
